What You’ll Do
The Quality Assurance Nurse supports the Director of Nursing (DON) and Administrator in ensuring clinical excellence, regulatory compliance, and continuous quality improvement within the skilled nursing facility. This role plays a key part in Hawaii State Department of Health (DOH) survey preparation, compliance with Hawaii Administrative Rules (HAR) Chapter 94, and oversight of the facility’s QAPI program. The QA Nurse performs audits, monitors quality indicators, analyzes trends, and collaborates with interdisciplinary teams to promote resident safety and optimal outcomes
Location: Hale Ola Kino by Arcadia
Work Schedule
Full-Time (40 hours per week): Sunday - Thursday, 9:00am - 5:00pm
Compensation
Annual Salary Range: $81,200.00 - $101,700
Responsibilities
Ensures compliance with the regulatory agencies and facility policy/procedure through early identification/auditing of clinical events and trends, which require measurement to ensure problem reduction. Areas to be audited include, but not limited to:
MAR/TAR (EMR)
Order entries
Assessments completion and accuracy
Infection control practices, residents' rights, and skilled care protocols.
Clinical Audits and Staff Development
Perform regular audits of charts, assessments, MAR/TAR, care plans, and care delivery.
Support competency checks for clinical staff, including new hire onboarding and annual reviews.
Assists with Quality Assurance Process Improvement (QAPI) data entry to monitor, track, analyze, summarize, and provide recommendations and present reports to committee. Reports to include:
Medication Management – summarize & incorporate pharmacy consultant monthly reports
Medication Administration (EMR) – summarize & incorporate process improvement above threshold.
RTA – summarize data as it relates to residents who have RTA (returned to acute), reasons for RTA, and opportunities to avoid RTA.
Medication Errors – summarize & incorporate process improvement above threshold
Interdisciplinary Collaboration
Participate in interdisciplinary care team meetings, care conferences, and committee meetings (e.g., QA, Infection Control, and Safety).
Act as a liaison between clinical staff and administration to resolve compliance-related concerns proactively.
May serve as alternate or backup Infection Preventionist, supporting the infection control program.
Assists with analysis, summary, preparation and presentation of data to the facility Quality Assurance Committee on a quarterly basis.
Communicates and interacts effectively and tactfully with the resident, visitor, families, peers and supervisors.
Assists in maintaining a safe, neat and clean environment.
Additional duties and responsibilities:
Cooperating with other disciplines in providing continuity of care which includes assisting with nursing team on resident admissions, and being support staff during survey process.
Participation in resident/family education, when appropriate.
Assists in developing and maintaining nursing service policies and procedures to implement the standards of care, in conjunction with Director of Nurses.
Maintains clinical skills through nursing education and attends in- service training and education sessions, as required and needed.
Attends scheduled nurses’ meetings and monthly QAPI, as necessary.
Assures proper functioning of equipment and adequate supplies.
Position is typically Sunday – Thursday (9-5), but needs to be flexible to work weekends, evening and nights, as necessary.
May fill-in as a charge nurse or medication treatment nurse in an emergency coverage.

Education/Experience
Bachelor's degree (B.S.) from four-year college or university and a minimum of 2-3 years’ experience and/or training; or equivalent combination of education and experience.
Certifications, Licenses, and Registrations
Current State of Hawaii R.N. license and First Aid/CPR certification.
Computer Skills
Proficiency in using a computer including Microsoft Office Suite (Excel, Outlook, PowerPoint, Word) and other applicable software applications. Capacity to learn and master other applicable software applications.
Physical Demands
While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. The employee frequently is required to stand, walk, sit, and taste or smell. The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl; push and pull. The employee must frequently lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
